Through the Community Health Needs Assessment (CHNA) which began in July 2012, the overall goal of Presence Resurrection Medical Center is to improve the health and well-being of the community with a special focus on the needs of the most vulnerable. The top four health priorities that resulted from the assessment were Chronic Disease Prevention, Mental Health Issues, and Navigating Resources/Senior Issues. A collaborative team of community stakeholders and partners are working together with us to provide these programs in the communities we serve.
Chronic Disease Prevention:
- Kids' Summer Feeding Program continues Monday thru Friday until August 29th from 12 noon - 1 pm
- Local Farmer's Markets
- Local Food Pantries
Mental Health Issues:
- The Ten Tips of Prevention for Parents (MCYAF)
- Presence Behavioral Health - FREE Assessments; groups for families, children and adolescents; and groups for adults.
